Below are links to the forms you’ll need to schedule your sleep study with Comprehensive Sleep Medicine. Please fax these forms with the required documents to us after you’ve completed them. Once we have received a doctor’s referral, we’ll contact you to schedule your sleep study.

What to Bring to Your Sleep Study
At our clinic, we strive to keep our patients as comfortable as possible to ensure we get the most accurate readings. That’s why hope to help them recreate their standard bedtime atmosphere. As a result, we ask that you bring items or materials make you comfortable, such as:
- Reading Material
- White Noise Machines
Please do not bring valuables or a lot of cash, cigarettes, cigars or pipes. Smoking is not permitted at the sleep center. To achieve the best results with our assessment, we must request that you avoid weary any loose-fitting clothing or silky materials to bed. In addition, please come ready for your evaluation with all paperwork completed, your insurance cards, and a change of clothes for the following morning.
Adequate Sleep Study Preparation Makes for the Best Results
Please shower before the appointment, but avoid moisturizes or conditioners on your hair and skin. Electrodes will be placed on your scalp, so it’s crucial that you avoid these products to ensure we gather the most accurate readings. Toupes or weaves must be removed before your study.
As part of your sleep study preparation, we ask that you refrain from smoking, caffeine, and stimulants at least 24 hours before your study. Notify our staff if you have a tape allergy. All cancellations should be made at least one business day before your appointment. The alarm is set at 11:00 p.m. and turned off at 6:00 a.m. Please notify a specialist if you must exit the center between these times.
We respectfully request that you be ready to leave the sleep center 45 minutes after awakening. Our center closes at 7:00 a.m.
